Medication Therapy Management (MTM)

Medication Therapy Management, as is accepted by the pharmacy profession, is a service or group of services that optimize therapeutic outcomes for individual patients.

Under the Part-D medication modernization act, covered beneficiaries, who meet certain criteria of chronic disease diagnoses and out of pocket expenses, are eligible to receive a comprehensive MTM review by their pharmacist. Industry leaders are suggesting the completion rates of MTM cases will be a measurement for performance networks. As we move forward in the profession, opportunities like these will become even more important-and we want to be ready and proud of our network.

Our membership varies widely so we understand a large component of our program must be dedicated to educating our members on the importance of MTM completion. We offer very individualized sessions with our members to familiarize them with the platforms used for MTM services. Everyone has different levels of understanding of MTM’s and we know we must adjust our message accordingly.

Through APCI Choice Clinical and Claims Management, we are able to track completion rates of our members with the online platform Mirixa. This allows us to make sure our members and our network are completing their cases successfully.

Through a ‘Pharmacy Services Permit,’ granted by the Alabama State Board of Pharmacy, APCI has been able to develop an assistance program using the knowledge and skills of fourth year pharmacy students. This program allows us to provide MTM completion services to our members who have outstanding cases needing completed. This allows us to maintain the integrity of our network and ensure we are serious about our members’ performance ratings.
