As a valued Provider, periodic recredentialing is required to keep your pharmacy information up-to-date and accurate. Providers must comply with credentialing and quality management required by CVS Caremark®, and Provider must provide CVS Caremark with documentation and other information that may be needed in connection.

Failure to logon and complete the recredentialing questionnaire may result in a $250 administration fee, increasing in $250 increments for subsequent non-compliant events. CVS Caremark reserves the rights to exercise all remedies available to it under the Provider Agreement, Provider Manual and applicable Law.
