As a valued Provider, periodic recredentialing is required to keep your pharmacy information up-to-date and accurate. Providers must comply with credentialing and quality management required by CVS Caremark®, and Provider must provide CVS Caremark with documentation and other information that may be needed in connection.

Failure to logon and complete the recredentialing questionnaire may result in a $250 administration fee, increasing in $250 increments for subsequent noncompliant events. CVS Caremark reserves the rights to exercise all remedies available to it under the Provider Agreement, Provider Manual and applicable Law.

 This recredentialing update should take approximately 15 minutes to complete.
