You have previously received the Medicare Part D Specialty Drug Reimbursement Addendum (or Addenda) to 2018 Medicare Part D Retail Performance Network Program ("Addendum"). The Addendum provided reimbursement rates, as well as terms and conditions, for dispensing certain specialty drugs for applicable Performance Network Program networks.
