Effective January 1, 2018, Diabetes Testing Supplies manufactured by Ascensia (formerly Bayer) will be the exclusive products for Commercial Covered Persons for Blue Cross and Blue Shield of Minnesota (BCBSMN) and Blue Plus.
Other diatetes testing supplies will be excluded entirely from the Covered Person’s formulary. Covered Persons may have a voucher for a new Ascensia meter at no cost to the Covered Person.
