The State of Texas Health and Human Services Commission (HHSC) added certain vitamins and minerals to the Medicaid formulary to be available through a pharmacy effective September 1, 2013. These products can only be provided to Medicaid clients 20 years of age and younger (through the month of their 21st birthday), and only for the conditions listed below. Providers have been advised to include the diagnosis on the written prescription or pharmacists may contact the Prescriber for this information. The age and condition restrictions are limitations of TexasMedicaid as outlined in the Texas Medicaid Provider Procedure Manual (TMPPM) and may be implemented by the Pharmacy Benefit Manager (PBM) as a utilization management edit.
