Corrections to communication previously sent on January 3, 2018. In the 2017 Annual FWA Training and General Compliance Training for Contracted Pharmacies section on page one:

• The FWA Training non-compliance fee is $750 for the 2017 Plan Year

• To receive credit for this annual training requirement, you MUST have completed the training by December 31, 2017 and submit the electronic attestation provided on our website before January 31, 2018.

If you completed your annual General Compliance & FWA training during 2017, you MUST submit the electronic Attestation provided on our website before end of day January 31, 2018. Failure to comply with this requirement will result in non-compliance with your Provider Agreement, and will subject you to a non-compliance fee of $750.00. Non-adherence to any of the provisions set forth in the Provider Agreement, which includes the Provider Manual, addenda, and other Caremark Documents may result in the termination of the Provider Agreement and other remedies available to CVS Caremark.
