The CVS Caremark® Performance Network Program™ will continue in 2018 to work with several valued external partners to provide pharmacies with performance tools which can help improve beneficiary adherence and aid in improving provider performance.

CVS Caremark has partnered with OutcomesMTM®, a Cardinal Health company, to provide pharmacies with a powerful online tool, the Connect™ Platform, to identify and document drug therapy intervention opportunities. OutcomesMTM is the national leader in the design, delivery and administration of Medication Therapy Management programs.

OutcomesMTM provides guidance and resources to assist in the efficient and successful delivery of Comprehensive Medication Reviews (CMRs), available on the Connect Platform, through which pharmacists generate Personal Medication Lists and Medication Action Plans to reinforce patient understanding and ensure coordination of care.
