When encountering a Reject 75 – “Prior Authorization Required” on a claim for a Kaiser Permanente
member, pharmacists should call the OptumRx Prior Authorization (PA) Call Center for resolution.
These claims should not remain in a rejected status and the PA Call Center is available 24 hours a
day, 7 days a week to assist with these resolution requests.
PA Representatives are on hand to assist with the following rejection types:
*Part D vs Excluded rejections
*B vs D rejections
*Hospice rejections
