ACCU-CHEK GUIDE BLOOD GLUCOSE METER is the only meter that Roche Diagnotics is promoting in the marketplace including sampling in physicians offices.

Should an AmeriHealth Caritas Louisiana member currently using an Accu-Chek Nano or Accu-Chek Aviva meter require a replacement OR should a member present a prescription for a first time Accu-Chek Nano or Accu-Chek Aviva meter, PLEASE CONTACT THE PRECRIBER AND ASK FOR APPROVAL TO DISPENSE THE ACCU-CHEK GUIDE METER AND RESPECTIVE ACCU-CHEK GUIDE TEST STRIPS IN ITS PLACE.

PRESCRIPTIONS FOR TEST STRIPS FOR MEMBERS WITH EXISTING ROCHE METERS WILL CONTINUE TO PROCESS AND PAY.
