For any specialty and nonspecialty products not specifically addressed, the discount shall be set at the Pharmacy’s retail brand or generic rate as set forth in the applicable Exhibit B(s) or AWP-15.25% for brand products and AWP-40% for generic products. All fees include normal supplies required for administration, except supplies for home infusion services including pumps and tubing, for which a per diem rate shall be charged. All fees include shipping by common carrier. Should a patient request special shipping requirements (expedited shipping, Saturday delivery, etc.), the patient shall be charged for the and delivery charges cost differential.
