Kentucky MAC Notification – Granted MAC Appeals

In accordance with Kentucky MAC law (Ky.Rev.Stat.Ann. §304.17A-162), the following is a list of products for which CVS Caremark® MAC adjustments have been made with retroactive effective dates as a result of granted MAC appeals. Pharmacies that filled prescriptions on or after the retroactive effective date (i.e., the Initial Date of Service) for patients covered under the same health benefit plan can reverse and resubmit the claim in order to receive payment based on the adjusted MAC.
