Dear Alabama APCI member,

In an effort to help better understand patient behavior, APCI is working with the Alabama Quality Assurance Foundation (AQAF) on a brief patient survey as part of the "Know Your Meds Alabama" campaign. The goals of the survey are to learn how patients manage their medications, which healthcare providers they ask for advice, and what kind of relationship they have with their local pharmacy.

We ask that you offer this survey to your patients as they come into the pharmacy or as they are waiting for their prescriptions. The survey is available for patients to complete online at www.aqaf.com/kyma or as a printed survey that you can download and print here.
