CVS/caremark administers the prescription benefits for Ohio Medicaid plans
offered by Molina Healthcare of Ohio, CareSource Ohio and Paramount.
Effective July 1, 2015, the RXPCN will change from ADV to MCAIDOH for all
Ohio Medicaid Plans. Some members may not receive new ID cards. If your
pharmacy receives the following or similar reject for any Ohio Medicaid
members, please reprocess the claim using the updated RXPCN
