The generic for Azilect recently became available in the marketplace; however, effective February 1, 2017, the plans listed below will continue to cover the brand name Azilect at the Tier noted below.

The generic (rasagiline mesylate tab) will NOT be covered for these plan members. Please continue to dispense Azilect as prescribed rather than substituting the generic.

The generic for Zetia recently became available in the marketplace; however, effective December 23, 2016, the plans listed below will continue to cover the brand name Zetia at the current tier in 2016 and the Preferred Brand Tier in 2017.
