IMPORTANT INFORMATION REGARDING

Recall of Amikacin Injection by Teva Pharmaceuticals

On July 29, 2016, Teva Pharmaceuticals announced a recall of some lots of amikacin 500 mg/2 mL and 1 gram/4 mL injection because of the potential presence of glass particulate matter.

Per Teva Pharmaceuticals, the use of or exposure to the recalled product may cause adverse health consequences, and the probability of serious health consequences is likely low.

OptumRx notified members who may be affected by this recall. These members were provided information to help them identify if their medication is being recalled. In the future, these members may inquire about the amikacin injection prescription(s) you dispensed.
