Starting June 1, 2017, all UnitedHealthcare Community Plan Medicaid and Florida Healthy Kids members will receive new identification (ID) cards.

• All pharmacy claims must be submitted with the member’s new ID number.
• Claims submitted with the old ID number will reject as: NCPD reject code 52 – Non-matched cardholder ID and will need to be resubmitted.
• The reject message will include the new ID number to use when resubmitting the claim.
• If the member has not received their new ID card, please call the OptumRx Pharmacy Help Desk at 877-305-8952.
