Effective November 2, 2016, Heritage Summit Workers Compensation will have a new claim number format. The current six (6) digit claim number will be replaced with a twenty (20) digit number that includes hyphens. Please note the numbers, hyphens, and the Date of Injury must be entered for each claim. If you need assistance processing claims for Heritage Summit please call 866.672.3063.
