Effective January 1, 2018, Xerox GIS will split from current Plan Sponsor Xerox. Xerox GIS members will move from Rx Groups RX4112, RX4113 and RX4114 to RX0532.

CVS Caremark® is providing the following information to assist participating pharmacies in the 2018 Medicare Part D Retail Performance Network Program for Aetna Medicare Part D Plans.

The U.S. Department of Health and Human Services issued their final rule of Section 1557 to improve health equity under the Affordable Care Act (ACA). Under this rule, all covered entities are required to post a notice of consumer civil rights. You have been identified as a covered entity and are therefore required to post a notice of consumer civil rights.

As stated in the CVS Caremark Provider Manual, Medicare Part D Plan Sponsors must arrange with Network Pharmacies to provide Part D Enrollees with a written copy of the standardized pharmacy notice when a Part D Enrollee’s prescription cannot be filled under the Medicare Part D benefit at the point of sale. 42 C.F.R. §§ 423.128(b)(7)(iii) and 423.562(a)(3).
