On December 6, 2016, Baxter announced a recall of one lot of fluconazole 200 mg/100 mL solution for injection because of customer complaints received for the presence of leaks. Per Baxter, a leak in the fluconazole solution bag may delay therapy, under-deliver therapy, cause unintended drug exposure, or cause infections or other serious adverse effects. OptumRx notified members who may be affected by this recall. These members were provided information to help them identify if their medication is being recalled and advised to not use recalled medication. In the future, these members may inquire about the fluconazole prescription(s) you dispensed.
