On August 17, 2016, Sagent Pharmaceuticals announced a recall of one lot of oxacillin 10 g injection because of a product complaint for the presence of small, dark particulate matter, identified as iron oxide, found within the solution after reconstitution. Per Sagent Pharmaceuticals, administration of metal particulate to a patient has the potential to cause local swelling, irritation of blood vessels or tissue, or blockage of blood vessels. Blockage of blood vessels can lead to serious adverse effects such as organ damage. OptumRx notified members who may be affected by this recall. These members were provided information to help them identify if their medication is being recalled. In the future, these members may inquire about the oxacillin prescription(s) you dispensed.
