Your pharmacy currently participates in the South Atlantic Regional Network. The NetworkEnrollment Form is amended to reflect revised time periods for the effective dates, as reflectedin redline format below to demonstrate the change. All other terms reimbursement rates for thisnetwork will remain unchanged.

No further action is required and your participation in this network will continue unless you notify CVS Caremark® in writing no later than November 1, 2016, that you no longer wish to participate.

US Script and CVS Caremark® are pleased to announce that effective November 1, 2016, CVS Caremark will begin to process claims for Sunflower Health Plan. All claims for Sunflower Health Plan will be reimbursed according to your CVS Caremark Network Enrollment Forms and will be paid in accordance with the Kansas Department of Health & Environment requirements for KanCare Medicaid pharmacy claims. Sunflower Health Plan claims will appear on your CVS Caremark 835 or remittance advice.
